Clinical Practice
March 1976 - June 2004
Caroline Bowen
practised as a Speech-Language Pathologist in Gordon, NSW from March
1976 until June 2004.The Gordon clinic in suburban Sydney continues to thrive, and
details are
here.
July 2004 -
With an extensive clinical
background in all aspects of child and adolescent communication
disorders, nowadays in her clinical work Caroline is mainly
involved in the assessment and
treatment of children in the zero to five years age-group. Her
main clinical interest is
young children's language development and disorders, and
children's speech sound disorders, including developmental phonological
disorders, functional speech disorders, and childhood apraxia of speech
(CAS).
She is also interested in the
semantic and pragmatic difficulties, and the language input processing
issues associated with the autistic spectrum of disorders, and in
word retrieval difficulties.
Caseload: Children
0 to 5 years
Please note that Caroline's current
caseload mainly comprises children 0-5, and a few a little older
(Kindergarten, Year 1 and Year 2), who have communication difficulties associated with:
Childhood apraxia of speech (CAS)
Conductive hearing problems (otitis media / "glue ear")
Delayed language development
Delayed speech development
Developmental disability ("developmental delay")
Developmental
phonological disorders
Fluency disorders (stuttering, cluttering)
Functional Speech Disorders including
lisping
Semantic and pragmatic difficulties
Semantic
pragmatic language disorder
Specific language impairment (SLI)
Tongue-tie (Ankyloglossia)
Traumatic brain injury (TBI)
Voice
disorders
Word
retrieval difficulties
"Second
opinion" assessments
For many years Caroline's
practice has included a consultancy service to fellow speech
pathologists locally, interstate and internationally, providing
second opinions in cases of severe and/or persisting speech sound disorders. In
providing these assessments Caroline prefers to communicate with
each child's treating speech-language pathologist. About two thirds
of second opinions are initiated by treating SLPs, and the remaining
third by parents and caregivers.

Assessment
The assessment process for children
involves taking a detailed history from parent/s, gathering supporting information
(e.g., school reports, psychological assessment, audiogram), and assessing the
child's communication skills via
standardised measures and non-standardised clinical observations. Assessment usually takes
from one to three visits. Assessment reports (if requested/required) regarding children go
to parents for them to distribute at their discretion.
Reports
All reports go to
parents / caregivers for them to distribute at their discretion.
Therapy
The number and frequency of therapy
appointments for children varies according to the child and to the disorder. Parents are given
an indication of the amount of therapy likely to be needed as soon as possible.
An approximate
estimate of how much therapy will be required is usually possible after the
assessment is complete, and an even more accurate indication is
possible after the third or fourth therapy appointment.
Pre-school aged
children are seen during school hours, preferably on days that they do not normally attend
pre-school.
Although there are sometimes
problems associated with it (i.e., with regard to coming out of
school), it is preferable to see Kindergarten and Year 1 and Year 2 students
for appointments before mid-day, especially if they have attention
span problems or if they tire easily, as so many speech and language
impaired and learning disabled children do.
Fees
and insurance
A
current schedule of fees is available by regular mail on
request. Fees are always
discussed with clients prior to the initial consultation.
Speech-Language Pathology fees are GST exempt and are partially
covered by private health insurance funds, but not Medicare. Note
that most funds will not pay a rebate for report fees.
